Welcome to PMA Child Care Center. The documents below are provided for families who have completed a tour and are moving forward in the enrollment process.

Submission of documents does not guarantee enrollment. Placement is based on availability and licensing capacity.

Before downloading enrollment documents, families must have completed and in-person tour of PMA Child Care Center.

Enrollment applications are accepted after a completed tour and confirmation of availability. Placement is determined in accordance with Tennessee Department of Human Services (DHS) Child Care Licensing regulations, including staff-to-child ratios and licensed capacity.

Families approved for enrollment will receive Procare access to complete remaining enrollment requirements.

Parent communication, billing, and enrollment completion are managed through the ProCare Parent App. Access is provided after enrollment approval.
